Understanding Your Care Options
Levels of Care Explained
This page provides an easy-to-understand overview of the different types of care available for patients at every stage of their health journey. From hospital and rehabilitation services to skilled nursing, long-term care, assisted living, and hospice or palliative support, each section explains what patients and families can expect and how care is tailored to meet individual needs. It’s designed to help families make informed decisions and feel confident in the care options available.
Acute Hospital Care
This is the first level of care when someone is seriously ill or injured. Patients receive immediate medical treatment, tests, and monitoring to stabilize their condition
Acute Inpatient Rehabilitation
After a hospital stay, some patients need focused therapy to regain strength, mobility, or independence. Inpatient rehab provides daily physical, occupational, and speech therapy under close medical supervision.
Skilled Nursing Facility (SNF)
Skilled nursing centers offer short-term medical care and rehabilitation for patients who aren’t ready to return home. Nurses, therapists, and providers help patients continue healing and regain function.
Assisted Living
Assisted living communities support individuals who can live mostly independently but may need help with meals, medications, or daily routines in a safe, home-like environment.
Long-Term Care
Long-term care provides ongoing support for individuals who need help with daily activities or have chronic medical conditions that require consistent monitoring and assistance.
Hospice & Palliative Care
Hospice and palliative care are specialized approaches that focus on comfort, dignity, and quality of life for people with serious or life-limiting illnesses. Palliative care can be provided at any stage of illness to manage symptoms, reduce stress, and improve overall well-being. Hospice care is typically provided when recovery is no longer the goal, offering support and comfort for patients and their families while honoring their wishes and needs.
Smooth Transitions, Better Outcomes
Experience matters when it comes to guiding patients through the continuum of care. Research from the NIH highlights that “clear and comprehensive provider–patient communication, along with coordinated planning across post-care settings or home, is critical to improving patient outcomes and reducing readmissions.” By keeping care coordinated and families informed at every step, each transition is safer, more efficient, and focused on achieving the best possible health outcomes.
